Working to ensure the safety and effectiveness of your medications
Our Medication Therapy Management (MTM) program helps you and your doctor manage your medications.
If you qualify for the program, a pharmacist will review your medications and talk with you about:
- How to get the most benefit from the drugs you take
- Any side effects or reactions
- Any questions or concerns you have
- Options that may help you save money
We automatically enroll members in our MTM programs, if you qualify. Participation is voluntary. You can opt out of the program at any time.
What does it cost?
There's no added cost to be in our MTM program.

Do you qualify?
2021
You qualify for our MTM program if you:
- Have three or more of these conditions:
- Osteoporosis
- Chronic Heart Failure (CHF)
- Diabetes
- Depression
- Asthma
- Chronic Obstructive Pulmonary Disease (COPD)
- Cardiovascular Disorders (Not for Prescription Drug Plans)
- Are likely to spend more than $4,376 in 2021 on prescription drug costs (includes amounts paid by both you and the plan)
- Take eight or more maintenance medications that are covered by your Medicare Part D plan
If you meet all of these criteria, we'll mail you a letter confirming your enrollment in our MTM program. You'll also get a call from us.

Learn how the program works
2021
Once in the program, you’re offered a comprehensive medication review. It includes several steps:
- A pharmacist reviews all of your medications to see if there are any drug therapy issues.
- If needed, a pharmacist will work with your doctor to resolve any drug therapy issues.
- The pharmacist will talk with you over the phone or in person at your local pharmacy. They’ll review your current medications (both prescription and over-the-counter) and your medical conditions. This will take about 30 minutes.
- The pharmacist will give you a personal medication list and an action plan to help you manage your medications. Be sure to share this information with your doctor.

We’ll review your medications at least once every three months. After each review, we may send suggestions to your doctor about prescriptions that may be safer or a better alternative. As always, your doctor will decide whether to consider our suggestions. Your prescription drugs won’t change unless you and your doctor decide to change them. We may also contact you with suggestions about your medications.
MTM programs are not considered benefits.
